SDA Registration Pack
This pack supports NDIS Providers, who are delivering Specialist Disability Accommodation (SDA) only (i.e. not delivering any other NDIS supports or services) to apply for or renew their NDIS registration and undergo the required Certification Audit against the NDIS Practice Standards’ SDA Module. The Pack includes an SDA SmartPlan setting out the registration or renewal process and Policies and Procedures that meet all relevant Practice Standard requirements.
The Pack also provides free access to supporting SDA forms and templates, including a sample SDA Application Form and SDA Service Agreement. which will help businesses to complete the Self-Assessment required as part of their registration or renewal application.
